Travel with Mandy Terms & Conditions

Thank you for your business, it is important to me! Before I make arrangements for your flight, hotel, car rental, tour, cruise, or other trip, I require you sign this form; the signature will signify your agreement with the following terms and conditions.


Please take the time to read your terms and conditions. Agreement is required for the service of Travel with Mandy to further plan your vacation.  Please contact Mandy if you have any questions or concerns prior to signing. (920) 415-4977.


GOVERNMENT ISSUED IDENTIFICATION REQUIRED:

*DOMESTIC: When traveling within the United States, all travelers (age 18 and older) are required to present a valid (unexpired) driver's license or government-issued photo identification.  Any teen who has a valid driver’s license or state ID is encouraged to bring it. If a teen appears close to age 18 and may be questioned by airline staff, it is recommended to carry proof of age (such as a birth certificate).

*INTERNATIONAL: For international travel, a valid passport is mandatory for all travelers! Many countries require validity of at least 6 months beyond your planned return date.   

*Minors traveling internationally should— and in some countries may be required to — provide proof that both legal parents or guardians are traveling with or have consented to the child’s travel. This may include a notarized consent letter or other official documentation, depending on the destination’s regulations. If a child is not traveling with both parents listed on their birth certificate, you must notify Travel with Mandy at time of booking.

*It is the responsibility of the traveler to ensure they have the necessary required travel documents for their destination. Failure to comply with these identification and documentation requirements may result in denied boarding or entry at immigration checkpoints. Denial could result in a forfeiture of all monies paid.

*It is imperative that travelers adhere to these guidelines to avoid any disruptions or delays in their travel plans. Refer to www.Travel.state.gov – International Travel for country specific requirements.


DENIAL OF ENTRY WARNING: Certain countries may deny entry to travelers with even a minor criminal record, including past DUI convictions, regardless of how long ago they occurred. If you are not a U.S. citizen, re-entry to the U.S. may also be restricted. It is the traveler’s sole responsibility to confirm entry eligibility with the U.S. Department of State and the embassy or consulate of each country being visited. We do not accept responsibility for denial of entry, missed portions of travel, or any resulting cancellation penalties.


AIRLINE RESERVATIONS - All name spellings MUST EXACTLY MATCH the reservation. Airline imposed schedule changes are possible even after ticket purchase. We highly recommend you download any possible airline APPs for flight tracking and contact during travel. Please reconfirm your flights the day before any flights. Most airline tickets are non-refundable & non-changeable. Fees may be required for changes. Individual airlines establish their own terms of sale. If you do not understand the terms of your purchase, please ask for clarification before you authorize your purchase. Verbal agreements are binding, change fees are your responsibility.


LUGGAGE: Many airlines have instituted a policy of charging travelers for luggage (carry on & checked baggage). Charges vary according to individual airline policies. Most often those charges are payable directly to the airline at the time of check-in.

SEATS: Travel with Mandy will make every effort to get seat assignments as you request, however the airlines are allowed to make changes to their schedule and equipment at any time which could cause a loss of previously assigned seats. Not all airlines allow seat assignments in advance, some airlines will charge additional fees for advance seat assignments.
 

ACCOMMODATIONS: At the time of reservations all known required taxes & fees are advised, however these are subject to change at any time.


RENTAL CARS: A major valid credit card & valid driver’s license in the renter’s name are required on all car rentals whether it is on a prepaid voucher or not. Debit cards are not always accepted in all locations due to the limited amount of money available per day. It is advisable to check to see if your personal auto insurance policy applies to rental cars.  International rentals may require an International Drivers License or further documentation.  


REFUNDS: Availability of refunds for unused travel reservations will vary with the type of reservation and the point at which reservations were not used. Service fees for refunds may be applied. Please note that when you purchase travel services, your money immediately belongs to the supplier. In the event of any supplier bankruptcy, you should seek refunds directly from the supplier or bankruptcy court. All fees charged directly by Travel with Mandy, for service given, are non-refundable.


TRAVEL WITH MANDY SERVICE ROLE: Travel with Mandy acts as a sales agent for any airline, hotel, car-rental company, tour operator, cruise line, or other service provider named in your itinerary (“Suppliers”). TRAVEL WITH MANDY (“TWM”) is not responsible for acts or omissions of the Suppliers or their failure to provide services or adhere to their own schedules. TWM assumes no responsibility for and shall not be liable for any refund, personal injury, property damage, or other loss, accident, delay, inconvenience, or irregularity which may be caused by: (1) any defaults, wrongful or negligent acts, or omissions of the Suppliers; (2) any defect in or failure of any vehicle, craft, equipment, or instrumentality owned, operated, or otherwise used or provided by the Suppliers; or (3) any wrongful or negligent acts or omissions on the part of any other party not under TWM’s control. You acknowledge and understand that: (1) airlines can increase their prices at any time before tickets are issued and that TWM has no control over issuance of tickets for tours; (2) airlines, cruise lines, tour operators, and other Suppliers have their own contracts covering cancellation penalties and other terms and conditions, and that you may be bound by those contracts regardless of whether you receive notice of their terms. By signing below, you are consenting to those terms and conditions. TWM has no special knowledge regarding the financial condition of the Suppliers, unsafe conditions, health hazards, weather hazards, or climate extremes at locations to which you may travel. For information concerning possible dangers at destinations contact the Travel Warnings Section of the U.S. State Department at (202) 647-5225 or www.travel.state.gov. For medical information contact the Centers for Disease Control at (877) FYI-TRIP or www.cdc.gov/travel. You assume full and complete responsibility for checking and verifying any and all passport, visa, vaccination, or other entry requirements of your destination(s), and all conditions regarding health, safety, security, political stability, and labor or civil unrest at such destination(s). You hereby release TWM from all claims arising out of any problem covered in this paragraph and submit all other claims against us within 30 days after the return of your trip. You agree the courts in Outagamie will be the exclusive jurisdiction for all claims brought by you or TWM, and you hereby submit to the personal jurisdiction of those courts. For your protection, we strongly recommend that you purchase trip cancellation insurance. We also strongly recommend that you use a credit card for your purchase, so that you can exercise your rights under the Fair Credit Billing Act if you do not receive the services you purchased. As our client, you hereby appoint us to be your attorneys-in-fact for the purpose of signing all documents necessary to purchase and issue airline tickets, tours, cruises, and hotel guarantees for late arrival. You agree that you will pay for all such purchases and will not hold us responsible for any of our actions pursuant to this power of attorney. You hereby release TWM from all claims arising out of any problem covered in this waiver of liability.
